Borrowed from decision-making psychology, a pre-mortem asks your team to imagine it’s six months in the future, and the project has utterly failed. Then ask: "What went wrong?" This isn’t about predicting the future, but about forcing your team to identify potential failure points without the constraints of optimism bias.
- How to run it: Dedicate a focused hour with your team. Frame it as a learning exercise, not a blame session. Encourage wild ideas and "what if" scenarios. Document everything.
- Example: "Okay team, imagine it's six months from now, and this new feature launch was a disaster. What caused it? Let's brainstorm all the possible reasons, no matter how unlikely."
2. Probability & Impact Assessment:
After the pre-mortem, don’t let the ideas languish. For each identified risk, collaboratively assess:
- Probability: How likely is this to happen? (High, Medium, Low)
- Impact: If it does happen, how severe will the consequences be? (High, Medium, Low)
This isn’t about precise calculations. It’s about prioritizing. Risks with High Probability and High Impact demand immediate attention.
Integrating Risk Identification into Your Workflow
This isn’t a one-time exercise. The goal is to weave risk identification into your regular processes:
- Sprint Planning: Include a short “risk assessment” section in your sprint planning meetings. What potential roadblocks could prevent you from achieving your sprint goals?
- Code Reviews: Beyond functionality, encourage reviewers to consider potential security vulnerabilities, performance bottlenecks, and maintainability issues.
- Architecture Discussions: Challenge assumptions and explore alternative approaches. What are the trade-offs? What could go wrong with this design?
- Post-Incident Reviews: Don't just fix the immediate problem. Dig deeper. What systemic issues contributed to the incident? What can you do to prevent it from happening again? (Jin et al, 2007 highlight the value of root cause analysis for knowledge elicitation, which helps uncover hidden risks.)
The Human Factor: Risks Aren't Just Technical
It’s easy to focus solely on technical risks, but remember that human factors are often the biggest contributors to project failures. Are key team members overloaded? Carefully planning and adaptation can reduce uncertainty, but equally important is fostering a team environment where concerns can be openly discussed and addressed.
Boldness Over Triviality
It's tempting to focus solely on avoiding risks. But remember, risk is inherent in innovation. As the saying goes, it’s better to risk boldness than triviality. The goal isn’t to eliminate risk entirely, but to understand it, mitigate it where appropriate, and make informed decisions.
